I would recommend oxygen therapy. Have you tried hyperbaric oxygen?
I tried to find info on fistulas and hyperberic oxygen, but this is all I could find.

You could also use an ozone generator pointing the hose toward the fistula or maybe try cupping the area? Or you could try H202 baths. I would get oxygen to the area somehow.
