Start exercising every day. Something that can be done comfortably without pain so as to make it a habit. When I recovered from my back surgery, the very first walk I did was out to the end of the driveway and back. With in 2 months I was walking that many miles... 2 more months and I was walking 7 EVERY DAY.

Think of exercise as your medicine. Exercise alone takes the place of too many pharmaceuticals to list.

Next, start fixing and creating healthy meals together... learn to shop for the correct types of produce, salt, oils, grains, dairy and meats if you eat them, and sugars.

That would be the best start to changing habits... after that is established, start thinking about the cleanses based on how the body responds to the above. Of course quiting the tobacco may be on the radar too, but take it one slow step at a time as tolerated AND to develope the changes as lifetime habits.
