Sorry to mislead you but HOW WEIRD!! Do they say why they wont send you the results? They make it sound like if you fill out the requisition form you don't need to involve the doctor.
I used a lab in the UK called Parascope (part of the Leeds Teaching Hospital). I did quiz the technician about the necessity of a doctor's involvement but didn't get a satisfactory answer. I had the feeling the guy wouldn't have agreed to send the results to the vet if I hadn't been so persistant
