Yes, in some cases it can be managed homeopathically. Every case is different. The vet who mentioned surgery was a homeopathic vet- don't know in what way surgery would be applicable- it was just mentioned as a last- resort alternative. No elaboration. The Dr. Pitcairn book you mentioned( as well as others out there), does give helpful advice for treating it homeopathically but sometimes in acute cases it just cannot be managed at home especially if the person does not know how to give subcutaneous fluids. Sometimes intervention is needed to get it under control, sometimes it can be managed at home. A tough call, but only the pet owner can make that decision.
