If you are having all those problems, I would suspect you're consuming refined foods. Refined grains (e.g. pasta), refined sugars (e.g. sweeteners in soymilk, fiberless juice), and dairy could all be to blame.
If you aren't already doing so, try only eating whole plant foods: vegetables, fruits, whole grains (especially brown rice), seeds and nuts. This is a very basic diet that is often life-changing.
