Any advice to help 80 y.o. grandma w/ faltering health, in need of immediate help

I live with and take care of my 80-year-old grandmother. She has many structural and digestive problems and her health is going down fast. She's in so much pain that she is reluctant to try many "alternative healing" ideas. My family doesn't know who to ask. Doctors don't seem to care about preventive care for elders, about educating about diet, discussing options, etc. They seem to think, "Who cares. They're going to die soon, anyway."
Here are my concerns:
1) She takes pain pills, synthroid (tho' her former chiro discussed switching her off it, he never followed thru), meds for acid reflux, allergies & athsma, and others I'm forgetting. I'm sure these clog her liver and add to constipation and fatigue.
2) Her diet is horrible and she has chronic constipation. Though I've told her repeatedly she needs to get off of chocolate, coffee, refined flours, cheese, etc. I don't really know how to advise her.
3) She's had scoliosis, all her life. Has had 2 lower back surgeries to fuse disks. She has sciatica. She has osteoarthritis in her spine. She lives in chronic pain, debilitating pain everyday. She's a proper woman but cries out in pain when she moves.
4) Her face has precancerous spots, tho' recently treated. Her fingers can't paint or knit anymore. Any solutions?
My questions:
1) Is cleansing different for seniors? Bowel clease/liver cleanse/gallbladder cleanse/others?? If she changes her diet for the better, how should she proceed so she doesn't suffer horrible reactions and get sicker. I understand death is even a possibility from cleansing then returning to a "regular" diet and I fear I may give her bad information. Who can help.
2) How much is diet contributing to her chronic pain? How much are meds contributing to her digestion problems?
3) Are there any experts out there that assist seniors in changing diet safely after a lifetime of poor diet and taking meds? That can help w/ re-education, and help w/ the psychological stress of making necessary changes?
4) Is anyone else trying to help their grandparent feel better w/ a medical system that doesn't help much?
We've tried: MDs (for usual meds), physical therapists, chiropractors, massage therapists, jin shin, some accupuncture.
Please contribute any and all experiences, ideas, facts, theories...
