I would start w/cleansing. A Bowel Cleanse utilizing intestinal formula #1 and #2. I would progress into the five day liver cleanse.

Eliminate dairy, meat (at least go organic and limit it), eat fruits, veges, raw seeds and nuts along with whole grains.

I would start taking Vitalherbs http://superfood.andmuchmore.com/

BFC on the area for sure. Hot cold showers for sure. Massage for sure. I am a fan of chiro but I don't like the things this one has said to you. I might look for another. Accupuncture is another possibility.

Depression becomes a part of the disorder for sure. I'd get a St. John's Wort product at http://continue.to/organicherbs I'd also get Kava Kava Skullcap Valerian to help w/sleep. To me, getting sleep back on track is extremely important. Two anti-imflammatories from organicherbs come to mind - Turmeric (the next great herb in my opinion) and ginger - all of these can be pushed by Cayenne!

You also want an EFA supplement - I prefer Udo's Choice Oil from a local health food store that refrigerates.
