Richard Schulze's Tooth and Gum Restore Formula

Hi, I was looking at this:

And it doesn't explain how to make it. It only gave solid ingredients. Is it a tincture? with alcohol? where could I get barberry root bark, oak galls, Echinacea root, Tea Tree oil, and Peppermint oil?

If you only know an answer to one of my questions, please speak up. I have to get offline right now so I can't look.
