It's probably
food allergies . You mentioned that your daughter has a very limited diet. This is fairly typical for a child with allergies. Unfortunately, it's usually one or more of the foods in that limited diet which is the big rash generator.
We have friends who we got to try eliminating various "foods" (processed crap, actually) from their children's diets, and they eventually discovered that eating corn, corn products, corn chips, corn syrup, or anything else corn-related results in the outbreaks.
It takes some discipline (and willingness to endure the begging and pleading from resourceful three-year-olds who want their fixes of their favorite allergic foods) to pull this off, but if you are willing to work at it, you will find it.
Once you find one food or set of foods which cause the problem, you can prove whether or not it's the cause by eliminating it from the diet for a few weeks, and then letting the child eat some and note the result. It shouldn't take more than two hours for the rash to break out after the body has missed it for a few weeks.
I suggest consulting a naturopath who has worked with
food allergies (this is virtually all of them, BTW) to assist you with this process. Get out of the "mainstream" (*spit*) medical system, because it will never help you find the cause of this.
