It depends where you live.
I believe some states have a philopophical exemption.
Mississippi and West Virginia only have a medical exemption because "no one has challenged" the religious thing at the state level - yet.
I live on a military base and so I am not protected by any particular state. I asked several people and everyone told me there was no way I was going to get an exemption and it's never been done here before. I even ran into one woman who had looked into it before and finally gave up and just got her kids vaccinated.
My daughter has some special needs (my children were vaccinated until 2 years old). I went to the doctor to try to get a medical waiver, thinking that was my only option. I was refused a medical waiver because doctors don't believe that if your child is special needs and had gotten ill after their most recent
vaccination that there could be any relationship between the two. He said her reaction to past vaccines was "within known acceptable parameters".
BUT, the doctor was able to give me a piece of paper that said that he counseled me about
vaccinations and that I stated that it was against my religion to vaccinate. He said that he didn't know if it would get me anywhere at the school.
I had the paper notarized and walked into the kindergarden and preschool registration and just handed it to them. Behold, they do accept religious and/or medical waivers - but not philosphical. They did not ask me what my religion is.
As far as I am concerned, even if you do not belong to an organized religion, the beliefs that you hold dear about not harming your children are part of your own personal religion, as I believe everyone has a personal religion.
