It sounds to me like you are doing a great job! A couple things to try...
Have you tried finding raw goats milk? Check with your local feed store if you have a problem finding farms. Ask who they sell goat feed to and for there contact info. Raw tastes better and with no odour :). Can you pump breastmilk? If so, try adding some omega 3 fish oil to it. Be sure to find the purest form - no heavy metals and NOT concentrated. I like
http://www.innatechoice.com. Try mashing up some avocados and apples together - sweet and good fats too. Always cut your juices with water for the little one. Juices digest very quickly and typically go through the stomach right into the Small intestine to be digested. If there is any food in the stomach when juice goes in, it may make for excessive gas and hiccups. try keeping juice and food separate and see if that helps.

Keep in mind that the largest and strongest animals in the world (elephants and gorillas) are both vegetarians. I am not a vegan and do not claim you or your child should be, but we can obtain plenty of protein from these sources.
I wonder if he would benefit from craniosacral therapy. His little diaphragm may be the cause of the problem. A CS therapist can help unwind some of the tension very gently. This tension develops as the child does the natural corkscrew movement through the birth canal.
