I can try to answer some of your questions...
1) whole lemons are fine, as long as they are organic. If they aren't, cut atleast the outside of the skin off. The pith is great. You don't want ANY pulp, so strain it if your juicer leaves some in. The pulp means work for your digestive system.
2) Pepper: I've never heard of it, so I'm not sure. But it's not just about the heat. Is there some way you can compare nutritional content? Vitamins and minerals are what you're looking for.
3) Many do not recommend mixing the P&B shakes with the MC. If you choose to, wait until your bowels are moving consistantly, regularly and often. Then start with one and work your way up. Don't let yourself get constipated! Also, drink a LOT more water because the psyllium will hold a lot of it, and you don't want to get dehydrated.
