It looks like a parasite that was attached with the teeth/hooks in the moth part. Not sure what kind of parasite. Flukes do attach but they are usually flat. Roundworms are cylyndrical but they do not attach. There are countless species and anything can infect humans.
Have you tried praziquantel with albendazole for a few weeks? If you have flukes, this should give you a clue.
