It looks like a callous to me. It could be from shoes that don't fit properly or your gait. Some people will curl their big toe under when they take a step if their shoes don't fit properly. I've had similar issues before, but it went away after a few months of TLC and pedicures. Do you have flat feet or a very high arch? Do you wear heavy boots? Do you switch up your shoes on a daily basis or do you tend to wear the same pair everyday?

It could also be fungal. I see no reason why someone couldn't have an infection on a specific area of the foot, especially if the skin had been previously abraded. Maybe it started out as a callous from a strange gait or poorly fitting shoes and turned into a fungal infection where the skin was already damaged.

I tend to stay from hyperbolic absolutes like Never and Always. Life doesn't often allow for such, except in the cases of death and taxes. ;-)

I would treat it as both a callous and a fungal infection. Try wearing lightweight shoes with good socks and treat for athlete's foot (i.e., Epsom Salt soaks, ACV, topical creams with antifungals, eat antifungals foods and keep your feet clean and dry.) Not sure what you meant by 'using conditioner', but I hope you are not applying hair conditioner to it. Try coconut oil instead, but make sure it's fully absorbed and feet are dry before putting on socks and shoes.

Or go see a podiatrist...
