Need help with diagnosis

Hello, this is my first post on curezone. I am a 21 year old female who has been having stool problems ever since last August, and it continues to get worse as the months pass by.

History: It all started when I had a sinus infection in June 2013. I took Amoxicillin and Prednisone together for about 4 weeks. Then a couple days after I finished the prescriptions, I had a painful diarrhea attack which left me feeling very dizzy, pale, and cold hands/feet. After that diarrhea attack, I have been experiencing non-stop constipation with hard pebbly stools, no matter how much fiber, water, or exercise I got. GI doctors ran motility tests, colonoscopy, and a hydrogen breath test; all of them were negative and pointed to slow transit.

My symptoms include:

- Severe constipation: Over the last few months, my peristalsis has gotten so slow. Fiber, water, exercise, magnesium citrate does not work!
However, there was this one week in March where I was not constipated and had almost normal looking, clean stools, except I saw little white dots on them. After that one week passed, my stools began getting slower and slower again, and more sticky.

- Stool becomes very hard/pebbly if I eat refined carbohydrates and high Sugar fruits. If I avoid those, my stool turns soft, sticky, lacking form, and has an oily look to it. Also, there is undigested food in my stool, even when I do chew my food.

- My periods were regular up until late February. It completely skipped March and almost April (I had a colonic done in late April, and then my period came back the next day!) However, my period is late again for May. I also experience these hot flashes, especially on my back..

- My vitamin D level was 17 in last October. I brought it up to 50 earlier this year. However, my vitamin D level keeps dropping even though I am supplementing and getting enough sun! It has dropped from 50 to 37 in just a few months.

- Muscle twitches, waking up during the night, small headaches, tingling in hands/feet

- Difficulty breathing through my nose

- High eosinophil count, elevated SIgA

- Experienced another painful diarrhea attack when taking Miralax and eating tortilla soup.

I already did a stool test from Doctors data, and they found no yeast, but a couple commensal bacteria and an elevated SIgA. I also did an Ova and parasite test, which was negative. However, I did do a Candida blood test, and was positive for Candida Parapsilosis. I am currently on Nystatin and probiotics/magnesium.
Also, on Monday this week, I drank a cup of Smooth move tea, and I saw this in my stool:

I am worried because it kind of looks like a worm, even though it is covered in mucus.

Does anyone think I could be suffering from intestinal worms as well? Does Candida even severe constipation and painful diarrhea attacks?

I definitely want to do a parasite cleanse, as well as rid myself of the Candida overgrowth. It is just very frustrating when I feel like my intestines aren't working properly.

Thanks everyone!
