Could be anything and can be a mix of things but pinworms in the nose and ears? Extremely unlikely if not impossible (not their cycle plus they can't make it over there). Now what might look like a pinworm... Might actually be larvae from Ascaris and other parasites. You did mention that you had some strong reaction to albendazole... Well that's the drug which have a good effect there. If you do decide to take albendazole just take it at least a week after mebendazole. Black brown specs? Could be anything. An egg?
