They're targeting the muscles, the pain is obviously caused by the nerves. The muscles are only reacting.
It sounds to me like whatever is triggering this is coming from the brain.
Sleeping is calming down that trigger.
I knew someone with MS who had to have her hipbones removed because they were uncontrollably spastic and causing her a lot of continual pain.
This is extreme, but once those nerves are out of control anything can happen.
