So, what is confusing? That it would not be reported even if Gallstones were present, or that they would not even bother looking because it may not be considered significant?

You should note that autopsies are looking for cause of death and unless badly infected the gall bladder would not be considered as suspect. Therefore it would not likely be examined very thoroughly.
