Yes you have a massive infestation, I think flukes/trematodes as well. You will definately feel better when you get rid of it.

Print these pictures and show it to you doc and put them in a jar and send to a lab to be sure. But don't be surprised if they tell you it's negative (happened to me)

I'd definately go for meds, it might be a little tough on the liver, but then again a lingering fluke infestation is even worse.
