Add molybdenum to the list of things to try. Not saying it will be a magic pill, but it may be part of a combo that will help.

possibly related to sulfites and acetaldehyde, especially if candida is a problem.

Any symptoms of hypoglycemia? Remember it can be related to low adrenal problems.

Hg and heavy metals can contribute to all of the above.
