Looks like a lot of drugs you tried. You already tried tinidazole as well as doxycycline separately to no avail. Dr. Cahill recommended paromomycin and doxy. You said "in my case tinidazole followed by paramomycin." but later contradicted yourself and said "I did take the doxy w/ the paramomycin per Dr. Cahill's recommendation." Which exactly did you do?

Here is a question: would you have been able to obtain the tinidazole and paromocycin prescription were it not for the diagnosis provided by Dr. Cahill? Were other doctors unwilling to prescribe these to you?
