Still if a man has a PSA over 40 there is a very strong chance he has deadly prostate cancer that has spread.
If the PSA is over 20 the probability of cancer is around 95%.
The math of those correlations holds up despite the occassional elevated PSA due to infection or inflammation.
Enlargement and some other factors can elevate PSA but not usually to a great extent.Infection and inflammation can elevate PSA to greater levels but thats rare and not sustained.
Most of the controversy aabout PSA has to do with lower level readings (almost always below 15).
PSA does not save lives because there isn't an effective treatment for the deadly form of cancer.Medication,chemo and radiation may extend life a few months but whats the point.Alternative treatments are another matter but traditional medicine does not even consider them.
PSA also does not save lives because the non deadly form of prostate cancer does not usually kill,so whats the point in detecting it or treating it....(other than to make the urologists rich).
