Yeah, that's pretty much a nightmare. Sorry to hear about your troubles. Do you have additional information on particular strains? Especially with regard to the staph and strep?

The high dose (12 g/day) vitamin C protocol worked well for me, as did bile supplementation. Bile salts and acids worked on the Prevotella. High dose Vitamin C goes a long way toward clearing out your small bowel.
Oxygenating compounds such as Magnesium Oxide can loosen and kill large numbers of anaerobes. It's miserable stuff though. Enzyme formulas such as Klaire Labs Interfase help the
Antibiotics work better by breaking up biofilms and making the microbes more vulnerable.
Lactoferrin is another useful supplement that assists your mucosal defenses.
Getting your digestive process working, from HCL, to Enzymes, to Bile would be square one for me. Next would be to work on sIgA. That has to be abundant and working to maintain mucosal immunity.
Meditation, breathing from the diaphragm, nose breathing, and changing/managing the stress response are underrated and powerful ways to restore mucosal immunity in the gut.
Meditation with a focus on relaxing the abdominal area and envisioning blood flow to the area was helpful for me.
Breathing from the diaphragm increases lymph circulation in the gut. Increased lymph circulation equates to increased circulation of immune components.
Also, I recently came across the idea of Phage Therapy. I don't have personal experience with it, but given the complexity of your case, it might appeal to you. It's expensive, but very interesting.
