You don't have to fast for cleansing. The liver stops making bile when you fast and you need the bile for the flush. It should still work though.
Yes and you should drink water people sometimes get dehydrated and become nausous.

And you can mix the ES with grapefruit juice or so. I make a tiny bit of ES with hot water (after it is cooled off :) ) and drink it in one swallow and follow it with a glass of water.
