"Didn't Shelton recommend always fasting till true hunger returns??? Thought read that. If so then thats highly impractical for anyone with a lot of bodyfat as they would not have real hunger return for many many months even on water. And that is simply not practical in the real world.

Now I think he mostly fasted sick not very overweight people then could maybe see fasting until return of true hunger as people with low bodyfat would hit true hunger likely much faster as its a sign to stop the fast for self preservation reasons and likely built in to us. Maybe when get down to a low enough weight can try fasting till true hunger."

Fasting duration does not center solely around body fat percentage.  It is very common to reach genuine hunger by running low on one or more minerals or vitamins before running low on fat.  This was true also in the one instance i reached genuine hunger at 29.67 days. It is said that often leaner people will have better mineral and vitamin stores than those with high body fat (generally better absorption/assimilation and in many cases more nutritious diet to contribute to these reserves) and as such, the former will often be able to fast longer than the latter.  As to people generally being leaner in Dr. Shelton's day i couldn't agree with you more.  For various reasons, obesity today is quite epidemic by comparison.

Pretty extreme analogy, don't you think? :).  Especially when JLuke plainly stated:
