If it's in your gut, it's ALWAYS going to be on your penis (or in your vagina, for the girls). You can keep it down (hygiene), but it won't ever totally go away until you kick it out of your intestines.
Careful with creams and lotions if you use condoms: many of them will degrade latex like crazy.
...nystatin cream? I'd opt for topical ketaconazole (cream) instead. Won't cause the nasty side fx observed in systemic (oral) use, but packs a lot more kick. Still won't help 100%, but it can get you down from rashes and cracks daily to the occasional slight redness and discomfort.
Prescription dandruff shampoo (also ketaconazole), used as a shower gel for affected areas, can make a bit of a difference too. Btw, if you go that route, lather it up all through your pubic region, from navel to thighs. Chances are, you've got more candida in the area than just the tip of your penis.
OTC dandruff shampoos may or may not do the trick. Really depends on whether they were formulated properly or contain things that exacerbate it as much or more than the pyrithione zinc keeps it down.
If you aren't circumcised, then, strangely enough, your own urine can supposedly help you: overextend your foreskin forward, clamp down on it with your fingers, and urinate a bit into the "pouch" you've made, then swill it around a bit under your skin using your other hand. Afterward, dump it in the crapper and continue your leak as usual. Rinse with water after.
Also, tanning beds/booths can give skin fungi a pretty nice a
SSKI cking. Sun's almost definitely better, but often simply not an option.
