I agree. It is a great question. In many states, without actually lieing, you will be forced, pressured, and eventually become a case file in the state or municipality in which you live if you don't comply. If you're attempting to lie for "philosophical beliefs", better have an MD sign all waivers or you'll definitely get harassed.
