Excellent question. I dont have a straight answer, and I don't think anyone has .
In my view, both feed on each other, and are not mutually exclusive. In addition, I am almost sure that within the next 10 years, there will be another etiology surfacing that few have considered so far seriously. I suspect the a strong component and underlying trigger for both pancrease and adrenal pathology will be inflammation and stealth virus type component, which not only serves as trigger but also as main root cause. The reason I say it is because I have many cases of advance and refractory adrenal fatigue that is not stress related in addition to many that is of course. I have use anti-viral protocols for example, with some success in selected cases where the history points to that. I think the key comes down to history.
Your question is a good one for the academic environment. In the clinical setting where all you want to do is to get better, it does not matter. Why? Regardless of where it starts, symptomatically you need to do the same things to make yourself feel better. Following a protocol to balance sugar, which really translate into insulin regulation and not just sugar regulation, is treating the symptoms only. hypoglycemia is a symptom and not a disease.
your energy will be better serve is focusing on working on the hypoglycemia in terms of day to day management of insulin curve , regardless of whether it is pancreatic sourced or adrenal sourced. The adrenal do seem to lead the pancreas in terms of trigger from a clinical perspective. As the adrenal recover, the insulin curve stabilizes, and hypoglycemia normalized.
I am not sure if this helps you or confuse you more. Adrenal fatigue is such a challenging entity because we only know less than 5% of the pathology today.
