I had what I thought was PCOS. In fact, many of my symptoms paralleled yours. Had elevated testosterone, DHEA, acne, hypoglycemia. Cysts appeared on my ovaries in my late 20s.
Saw an endocrinologist who performed a plethora of tests, including all the andro hormones. She concluded I didn't have PCOS.
In my case it has to do with toxicity, as when I started to work on detoxifying my body the PCOS symptoms began to subside.
Heavy metals wreak havoc on hormones, enzymes etc.. Do you have amalgams, history of braces? That would be something to consider. Also, I tested high for lead, which was unexpected but I'm working on chelating out those metals. Hormones are slowly starting to regulate themselves;Acne, hypoglycemia have improved. It's been noted that many people with Lyme's disease have a heavy metal toxicity.
I imagine you've looked at your diet as well -->
food intolerances , refined sugars, gluten allergy.
