Make sure you hold the negative handhold
I, and others, have made the mistake of putting the negative handle over the liver, not the positive one--it causes a "bite", so I thought it was really doing something, but here's what Parazapper says about it--make sure it's the positive handhold, not the negative one you hold over an organ like the liver.

I hold the positive terminal in my right hand and place the negative terminal on my belly under my right rib cage.

This is not a good thing to do according to the information that I have collected as well as theoretically. The negative electrode does not hurt
parasites and may even be beneficial to them. For this reason, I suggest, always keep the negative electrode at one of the four extremities.
The positive electrode is the effective one and may be placed next to any area of concern except certain areas where it may cause damage. The eyes may be negatively affected. The heart may be negatively affected.
