I received my CDSA results and they are similar to what you say:
Lactobacillus - No Growth
E.coli - 2+
Bifido - No Growth
No pathogenic levels of additional bacteria or yeast were detected. I suspect 3 months of broad-spectrum antibiotics may have wiped out the beneficial bacteria. My GP is surprised by this, and questions the validity of the CDSA test, but admits it is theoretically possible.
I am now taking some heavy probiotics (45 billion cfu per capsule 3x/day) to rebalance the flora. My naturopathic doctor doubts candida or leaky gut. I sent him this article which only deals with candida as it relates to the published research:

Then there is the question of this being a stomache acid issue. The low amino acid levels in the blood suggest the ample protein I consume is not being absorbed. The CDSA showed ample pancreatic elastase 1, so I assume I make enough protease. Where else then but gastric acid secretion? I did try the simple baking soda in water test a few times, a belch within 2-3 mins suggesting good reaction with the HCl acid. I never burped. Also, it was suggested my deeply ridged (very pronounced vertical ridges) fingernails, wispy thin hair, and oily face (a putrid white cheesy substance can be squeezed out of my nose and cheeks daily... a little gross, sorry!) indicate low stomach acid secretion.
Could low stomach acid, too, cause the good gut flora to be eradicated?
Other info from the CDSA: 5.2 pH, which is too acidic, suggesting low protein or rapid transit time. No measurable putrefactive SCFAs (not sure what this means).
