Hypertension is an aberration of blood pressure over prolonged periods disturbing the mechanism of the whole body, Diabetes is a disorder not only by itself but which can contribute to degeneration of any organ you can name in the whole body, increased lipid levels can contribute to the generous blood vessels supplying to the muscles of the heart being clogged and finally Stress which can not only contribute to our body not being able to take the rigmorale of the day but also mind being bogged down to lose the inclination to live to fight another day.
Well then we have put our mind, energies and line of research to ponder about the probable application of treatment to various disorders of the modern man. But if there is any nagging, irritating and painful ailment which is as common as the above diseases or probably more than them which afflicts almost half the population at one point or the other in their lives it is Haemorrhoids or Piles which is the more common of way of referring to the problem.
WELL THEN WHAT IS PILES?
• Engorged, enlarged blood vessels or veins in the anus and rectum being inflamed and painful are called as Piles.
• They are also known as enlarged or displaced anal cushions. These anal cushions help seal the upper anal canal and contribute to continence or control while passing stool.
HOW DO HAEMORRHOIDS DEVELOP?
Increased abdominal pressure that extends its effect on the rectal veins causes this problem.
The factors that would increase this pressure are:
▲ Repeated straining in order to have a proper bowel movement in people suffering from constipation and amoebiasis etc.
▲ In pregnant women when the foetus extends the pressure on major blood vessels in the abdomen contributing to pressure on the rectal veins can cause the same.
▲ In people who are suffering from repeated attacks of diarrhoea.
▲ Obese people are more prone to this complaint
▲ Occupations that involve prolonged standing, sitting and heavy lifting would contribute to this ailment.
▲ In some diseases like Cirrhosis of Liver etc. which tend to increase abdominal pressure.
▲ If one were to enumerate all the above causes we would realize that this ailment is a modern man’s disease.
▲ Children rushing to school more than an hour before to travel conveniently skip to pass stool to save time. Sounds ridiculous but true. Parents force on eating food rather than insisting on the child to attend toilet on the right time. This will lead to constipation and later the ailment we are discussing about.
▲ Diet is also another factor which can influence the same. With the advent of influences across the world the fiber rich diet is being ignored and the food habits are becoming more favorable to junk like pizzas, burgers and generous amounts of colas which push one towards constipation.
▲ Sedentary occupations like working on the computers, call centers, desk jobs. People involved in standing while working like Surgeons, teachers, traffic police, bus conductors etc. also put pressure on the veins causing stasis and later on haemorrhoids. Drivers are also more prone to this ailment.
▲ As a consequence of above factors there is distal displacement of the anal cushions with swelling and inflammation.
CLASSIFICATION OF HAEMORRHOIDS:
I Degree: Bleeding only.
II Degree: Prolapse (Spontaneously the piles recede back.
III Degree: piles prolapse and can be pushed back.
IV Degree: Prolapse of piles is permanent.
SYMPTOMS OF HAEMORRHOIDS:
♦ Bleeding, burning and itching are common symptoms of Haemorrhoids.
♦ Pain or ache around the anus or rectum which could be quite severe while passing stool and in some cases could last for a few hours after passing stool.
♦ Bleeding from the rectum which could be usually bright red not usually mixed with stool but tends to fall in drops at the end of passing stool.
♦ In case of External Haemorrhoids there would be a sensation of lump or swelling coming out of the anus while passing stool.
If it tends to go back it is second degree Haemorrhoids, if it has to pushed back manually it is the third degree and in case of fourth degree the lump always remains out of the anus.
♦ Sometimes blood tends to get clotted in the lump outside the anus and it is called Thrombosed pile which could cause a lot of pain even while sitting and disturbing the routine daily work.
♦ Itching in the anus and the rectum and this uncomfortable feeling could be there after passing stool or sometimes after sitting for longer duration.
DIAGNOSIS:
The very presence of bright red blood at the end of passing stool and in some cases the prolapsed lump is enough indication of Haemorrhoids.
Care should be taken to rule out any Carcinoma of colon, the incidence of which is increasing.
PREVENTION:
Preventing Constipation is essential while treating Haemorrhoids. This involves eating diet which is high in fruit, vegetables and high fibre cereals etc.
Drinking generous amount of water is very important.
It is pertinent that one should not strain to pass stool in case of Constipation. It is better to wait for the peristaltic movement to pass through.
Avoid becoming over-weight and try to lose weight if one is.
It is better to take up regular exercise which will help in relieving constipation too.
Discomfort in the area can be reduced by washing with warm water. In case of prolapsed piles which are painful application of ice pack would reduce the swelling and relieve the pain to a certain extent.
