I have Apnea and used to wake up gasping for air many times a night, then other nights maybe only once.
I am 66 and gained 100 lbs since age 52 to obesity. I am mildly asthmatic and now insulin dependent diabetic. My triglicerides were 8.2, blood pressure up, and cholesterol ... bad ok, good low.
I refused the mask as they irritate me, and the same with the mouth appliance. I did not particularly believe in the doctor and his treatment.
So this is what I did: first I cut back on fatty meats like sausage, ham, premium steaks, foods fried in anything but pam or olive oil.
Cruceferous (cabbage family) vegetables, fruit, fish, sweet potatoes, tomatoes, onions, hot peppers, oranges and grape fruits were an increased replacement for bread, pasta, and irish potatoes. Portion control was added with great effort and the 10 pm snack was limited to cherrios or grapefruit . The fourth meal of soup, sandwiches, ice cream or home baked goods was was stopped with difficulty. I do not exercise do to emphesema so weight loss is very hard to come by.
Removal of the late nite
Sugar promoting snack stopped the apnea with the exception of when i fall off the wagon and eat! My triglicerides are now 5.8, BP and cholesterol normal.
I am getting over a lingering cough flu that reduceded my food intake to the point of losing about 10 lbs in two weeks. My one time nightly insulin has been reduced to 66% and other nights 0% dependent on the days food intake. Close monitoring is mandatory.

I believe a much greater loss of weight with low
Sugar maintenance could cure the apnea and diabetes.
I will continue the effort, but eating and sex run neck and neck for my attention, and both are reduced at this age.
Somewhere there is a quality of life factor involved here and that would address attitude.
