About 15 years ago Bob Dylan came down with histoplasmosis, and word through the news grapevine back then was that it was likely due to smoking weed that was grown in some bad soil. And pericardial effusion often goes hand in hand with pericarditis. (I'd ask if he's a spelunker but that seems less likely.)

Could be Iodine deficiency /hypothyroidism as mentioned;

It could also be pericarditis as mentioned; which also would call for iodine, salt etc. IMO. Whenever there is an inflammation or infection I would start with the basic Iodine protocol. Large amounts of vit C etc.
