I have an 1" stone and like you, used almost every softener mentioned around here. It does not work for calcified stones.

Read also my follow up post about surgery and the different possibilities. Since I was sure that I had read about laser surgery, since one of my friends customers was going to do it, I still want to find out what the change was that UpToDate made this year in their update.
Since my daughter works there I just sent her a reminder to see if there is a way she can find out.

If I talk to my friend again, I will see if it is possible to talk to her customer, even if it is the doctor's name since I know which hospital he was planning to go.
