Need help with actuvating first survey.

I created a survey, but the process is very confusing. At the end, I'm expecting for it to say somewhere "Submit Survey", but I never saw anything like that.

So now my survey says "Under Construction" but I think it's done.

What do I do next?

Also, I was thinking I would like to change the title, but I don't see how. Is that possible. I would like it to say "Thyroid/Appendectomy Survey" rather than "Unscientific Thyroid Survey."
