It turns out that Dr. Holland (author of The Fungus Link) works in the Dallas metroplex, just two hours away from me. I just sent him an email requesting more information. I will post his answer here. I also offered to do a non-scientific study to determine if an antifungal diet protocol would indeed slow symptoms or make improvements.
