So check the box marked personal, say you can't have vaccines, due to your illnesses...that your body will not stand for it. Also make it quite clear, that you are willing to seclude yourself and stay home if there is any outbreak of disease, that should please them. Depending where you live, it may not even be law to force vaccine, just a way of pushing it...
