I feel for you. It sounds like you are in a lot of pain. I am always suspicious when a doctor diagnoses IBS, because I was diagnosed with it at one time but there turned out to be a real clinical reason why I had IBS symtoms (and it wasn't because of depression!). Some of your symtoms sound like either Chrohn's disease or colitis. I would suggest doing some research yourself and looking at symtoms of other intestinal problems besides IBS. You may be able to figure out what you have. Then, the next step is to convince a doctor and get the proper treatment. It sounds like you may need to change doctors to do that.
One thing you can do right away though is work on your diet. You don't mention what your present diet is. I'd suggest you cut out sugar, wheat, dairy, caffeine, white carbs, alcohol and tomatoes. Supplements that I've found helpful that may help you are glutamine (which helps rebuild the intestinal lining) and probiotics (to replenish the good bugs). I don't have the same symtoms as you do, and I can only say this based on all the research I've done, but zinc is supposed to help with both Chrohn's and colitis.
Good luck!
