Since there is such a wide array of newly-discovered pathogenic links to maladies, and these are often helped with a double course of Doxycycline, I'd hit the scrip trail. For example, I think Yersinia Colitica and the related Yersinia Paratuberculosis bonds with or binds collagen (per an abstract in an Arthritis journal). This is a rod shaped bacilli, and is Doxycycline susceptible.
Yersinia can become systemic, and have latent effects, like joint pain. So what if a theoretical chronic exposure or latent effect from contact with Yersinia-carrier farm animals, raw milk, meat or shellfish, is targeting the collagen of the unique lip tissue?
