Keep in mind my post is only suggesting an alternative theory.
There is obviously a neurotransmitter problem with anorexics and as you say the problem is complex.
I don't remember any study that showed serotonin levels were higher in anorexics (do you have a reference.)
There was a study where the researchers suggested anorexics might have higher levels of serotonin or greater effect because of problmes with serotonin receptors in anorexics.
But the faulty receptors go both ways impeding and increasing the function of serotonin.
I would suggest anorexics have problems with lower serotonin levels or problems with adequate serotonin functioning due to the common copresenting symptoms - anxiety attacks in particular - a classic sign of poor serotonin levels or release.
Also the obsessive compulsive behaviour of anorexics is also common to poor serotonin levels or functioning as is aggressive behaviour.
I do not agree with the theory that serotonin levels are high in anorexics.
High levels of serotonin simply do not produce agressive, obsessive compulsive, anxiety ridden behaviour.
It's also important to note that eicosaoids like PGE1 (made from the EPA found in fish oil and made naturally by your body) control the release and reuptake of serotonin and other essential neurotransmitters like dopamine and norepinephrine.
