Can't get to a doctor

With all my problems over the years there is one thing for certain. I won't go to a doctor unless I'm dragged to an emergeny room in severe pain. I suffered with IBS, and diverticulitus for three years. I now have that under control. Now...I'm suffering from a three year old anal fistula, and VERY painfull hemerrhoids, plus a fissure. I need to go to a doctor and get this fixed. I feel like I am confined to a wheelchair because i really cant do anything. Just sit, or lay all day an suffer.

I really need help..By the way, I have went to three theropists (sp) over the last three years and am now trying a hypnotist..NOT WORKING!! Can ANYONE help with this?
