See a urologist.

He will order an ultrasound.

Surgery is similar to the recovery time for a hernia. They make an incision in your lower stomach near/in your pubic hair and sever it there. You might be able to do it with 'sedation', which basically is anesthesia given at lower doses.

This also could be epididymititis. Doctor would also order an ultrasound here. They would also have you give a urine specimine to see if it is bacterial in nature.

Either way, see a urologist.
