HELP! Very painfull testicles!

I am suffering from a flare up of a chronic inflammatory testicular condition. It could be viral and the symptoms consist of inflammation, including veins, pain in testies but also in groin area. It seems to be connected with sexual energy, (aggravated by close contact with my wife). When I am not in too much pain the symptoms can be relived by self-gratification.

I have had the condition for a number of years and from time to time it flares up, but this time it seems really bad. A number of years ago I had an exploratory op and was told it was a urinary infection but subsequent treatment with Antibiotics proved ineffective.
