Thank you for your responses. Having contacted dozens of area churches with the standard reply being that they wouldn't write a religious exemption letter because they support vaccinations, my daughter abandoned seeking a consenting pastor.

She finally found a downloadable form on a Nation of Islam website referring to the 14th ammendment to the constitution, etc., and took it to the Health Dept. After some back and forth discussion, they did agree that the signed and notorized form was acceptable and issued her the required card. Tweet
