A friend of mine had this situation also and was on the list for a liver
transplant. I pointed her to the Dr. Clark protocols and she did the
following.
First the parasitical cleanse, with _at_the_same_time_ using the liver
herbs.
Then after the parasitical cleanse was done, she went to do liver cleanses
(1 per 2 weeks).
After 6 weeks she had an evaluation from her doctor, and he told her that
she would be taken off the list for liver transplantation! She was no
longer critical. He didn't want to know what she'd done, though, but who
cares. As long as she doesn't need to have that transplantation!
I told her to do a
kidney cleanse after she'd done her 6th liver cleanse,
though, because "just" being taken off the waitinglist is not really the
same as being in good health - yet anyway. And I think she should continue
to do
Liver Cleanses for a long time.

PS: milk thistle will not hurt your friend, but it will not by any
stretch of the imagination do the job.
