At question here are a couple of issues.

First, is this strain Antibiotic resistant?

Second and more likely is reinfection.

This is the thing to find out first, is why has this recurrence happened?

As for me, in addition to the Antibiotics , I would choose a good multi-frequency zapper of a good Rife machine.
