Painful lump that is my small intestine?

Hi all,

my small intestine seems to be in one hard lump which is painful to pressure. The area around the belly button a level deeper feels like one big hard lump, the size of a palm.

Any ideas? Could this be due to gas from SIBO? I'm currently on my 11th day of fasting, thought this should have resolved by now.
