A 45 day water fast. Has this been supervised by an experienced and qualified fasting supervisor, because if not this is highly irresponsible.
There is a Science and fine art to fasting safely and beneficially, so the symptoms you describe are more to do with pathology (starvation) rather than physiology (fasting).
She needs to be admitted to the nearest hospital asap for her sake.
