Once you have seen a few thousand with yeast infection, you come to realize that some cases just "can't" appear to be solved without a great deal of difficulty, eventhough the patient IS compliant and plays the game by all the rules. These are the "impossible" cases that are discarded by most all practitioners in teh end, and these patients can end up with severe depression. I've seen my share over the years and they can be a real challenge indeed. In some cases, the cause is hidden, but in other cases there is a lot more to the case than meets the eye, and there are underlying emotional causes.

The other point is (although much harder to accept for most folks) is that some people just don't appear to really want to get well, and they may have their own particular reason for this. A good book which I once read illustrates this point brilliantly, it is called "Love Your Disease" by Dr. John Harrison.
